Correctional Transition Support helps people re-establish stability, identification, and access to services after leaving correctional settings.
Our approach is non-judgemental, trauma-informed and person-centred.
Who this pathway is for
- People exiting correctional facilities
- Individuals without identification or documentation
- Individuals with health, mental health, or psychosocial support needs
- People requiring support accessing income supports
How we may help
- Support re-establishment of identification and documentation
- Assist access to income supports, including the Disability Support Pension
- Support NDIS access where relevant
- Coordinate health, mental health, housing, and community services
- Work toward reducing system barriers and supporting stability, safety, and community connection
- NDIS plan
- An NDIS plan is not required to reach out - this pathway supports people with and without one.
- Cost
- This pathway is delivered through Marra Bridge's existing supports - no-cost Case Management (subject to eligibility and capacity) or NDIS-funded supports - depending on individual circumstances.
- Who we work alongside
- We work alongside Corrective Services, community corrections, housing services, and any existing case manager or support coordinator.
- Service area
- We provide services across the ACT and NSW.
- Response time
- We aim to respond to referrals within 2 business days.
What this pathway does not control or guarantee
- Marra Bridge does not control decisions made by Centrelink, the NDIA, housing providers or other agencies - we assist with the process, not the outcome
- This pathway does not replace parole, corrections or justice-system case management
