What is Life Transition Support?

Life Transition Support helps people navigate significant changes - turning 18, leaving school, moving out of home, or a change in health or support needs.

Who this pathway is for

  • People turning 18 and moving from child to adult services
  • People leaving school and planning next steps
  • People moving out of the family home or into supported or independent living
  • People experiencing changes in health, mental health, or functional capacity
  • People transitioning between providers, services, or funding arrangements

How we may help

  • Provide clear guidance through each stage of the transition
  • Coordinate between existing and new providers, services, and systems
  • Support informed decision-making so people stay in control of the change
  • Work to maintain continuity of support through the transition period
NDIS plan
This pathway supports people with and without an NDIS plan.
Cost
This pathway is delivered through Marra Bridge's existing supports - Support Coordination, Specialist Support Coordination, or no-cost Case Management - depending on individual circumstances.
Who we work alongside
We work alongside existing providers, schools, family members, and any current support coordinator or case manager.
Service area
We provide services across the ACT and NSW.
Response time
We aim to respond to referrals within 2 business days.
