Case Management
What is Case Management?
Case Management is a holistic, person-centred support provided at no cost, subject to capacity. It is intended for people with disability or complex psychosocial needs who have very limited informal, community or formal support around them and who are at risk of falling through gaps between services and systems.
This is not a general case-management service for everyone. Eligibility is considered according to the person's circumstances, level of existing support, complexity and Marra Bridge's available capacity. There is no obligation to use another Marra Bridge support, although external assessments, reports or services may involve separate costs.
Case Management is not an emergency, housing-allocation, clinical or legal service. We help you work through health, mental health, housing, justice, income, and community challenges, working to bring it all together and reduce gaps between services.
